Rajitha Bommakanti, RN, BSN, NBC-HWC, CCM, FCN

I have been practicing nursing for over 30 years in various medical specialties. I currently work as nurse case manager and as national board-certified health and wellness coach.

In my nursing journey, I have seen gaps in patient care and the impact chronic medical conditions had on my patient’s quality of life. 

I wanted to make a difference, so I moved into a health coaching/transformation space where I am a health partner in educating, inspiring, and empowering people to make small changes to patient’s lives to improve their health. 

I am a habit change specialist.
